I hereby represent and agree as follows:
1. I am over 18 years of age; or, I am the parent or legal guardian of the subject of the workshop participant or client.
2. I wish to participate in A-Alignment. I understand the fee is $297.00 and non-refundable.
3. I am currently under the care of a physician or other health care practitioner.
4. I understand that no physician-patient relationship is established through my participation in workshops or sessions. I understand that this is not psychotherapy.
5. During a session or workshop, a list of available options or solutions will be described, but I agree that before undertaking any of these options or solutions, I will consult my physician or other health care practitioner whose care I am currently under.
6. I understand that Lotus Lantern Healing Arts (Liliana Barzola) and Alchemy Consulting (Bri Condon) and Rev. Josie Coleman reserve the right to end a participant's training at any time and will refund the client's payment in full.
8. I understand that what is taught in the workshop or session is not medical diagnosis, medical treatment or medical advice; therefore I will not use intuitive consultation to provide a medical diagnosis, provide prescription, treatment or psychotherapy.
9. When signing up for classes with Lotus Lantern Healing Arts (Liliana Barzola) and Alchemy Consulting (Bri Condon), I understand that the material presented does not in any way intend to fulfill licensure requirements or continuing education credit requirement for any professional credential.
10. I hereby release Lotus Lantern Healing Arts (Liliana Barzola) and Alchemy Consulting (Bri Condon), their agents, employees, successors and assigns, and their respective heirs, personal representative, affiliates, and any or all persons, liable or who might be claimed to be liable, whether or not here in named, none of whom admit any liability to the undersigned, but all expressly denying liability, for any, and all actions, causes of action, claims and demands of any and every kind and also any and all injuries and damages that may occur in the present or develop in the future.
11. I understand that I will be receiving energy work and that the purpose is for the well being of my heart and mind. I understand that there is no implied or stated guarantee of success or effectiveness of individual technique or series of appointments. I hereby acknowledge that this is not a substitute for medical care, examination or diagnosis. Lotus Lantern Healing Arts (Liliana Barzola) and Alchemy Consulting (Bri Condon)does not diagnose, give medical advice, make claim to cure, or predict the future.
